Shepherd of the Valley has an opportunity for a full time LPN Case Manager. This position will mainly focus on the Trumbull county area and coordinate case management from on-site hospitals to the facilities ensuring accurate information and efficient admission of residents . Only applicants with Long Term Care experience will be considered.

  • Informally direct the work of the interdisciplinary team members to promote cost-effective and appropriate care and to ensure services are provided in a timely manner.
  • Meet with administration, nursing staff, and any other departments to ensure interdisciplinary planning of managed care services.
  • Continuously follow-up on the implementation of managed care resident care activities and evaluation of outcomes, along with reassessment of the residents condition as needed.
  • Assist with the discharge planning team and social services as needed.
  • Maintain current case management records.
  • Assist in standardizing the case management process.
  • Interpret managed care policies and procedures as required. Maintain an adequate relationship with families, residents, insurance companies and hospitals.
  • Pre-certify all managed care admissions providing all necessary information as necessary to required staff.


  • Associate's degree (A. A.) or equivalent from two-year college or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience.
  • Must be a Licensed Practical Nurse in the State of Ohio.
  • Must be a currently licensed and insured driver with an acceptable driving record & reliable transportation.
