Overview

Shepherd of the Valley is seeking an experienced LPN/Assessment Unit Nurse to join our five start Howland campus.This position is responsible for the coordination of the MDS process, based on the comprehensive assessments of the resident's needs.

Responsibilities

The LPN Assessment / Unit Nurse communicates a sense of caring, concern and dignity for our residents and is responsible for:

 

  • Completes initial assessment of residents and all subsequent assessments.
  • Updates physicians orders as needed.
  • Responsible for entire MDS process.
  • Reviews MDS assessments when there is a change in a resident's condition and/or quarterly.
  • Develops Comprehensive Care Plan with the Interdisciplinary Team.
  • Makes recommendations for charting requirements.
  • Informs staff of any changes regarding resident care and care issues.
  • Monitors "RUG" scores for optimal reimbursement.
  • May be required to take on-call needs and assist with routine RN duties

Qualifications

MINIMUM QUALIFICATIONS OF  :

  • Must hold a Nursing Degree from an accredited program or college/university
  • A current, unencumbered, active Ohio LPN license and current CPR
  • At least two years experience in Long Term Care
  • Experience with Point Click Clare a plus
