The Home Health LPN Clinical Liaison coordinates case management from facilities/hospitals to home health ensuring accurate information and efficient admission of patients by performing the following duties.

  •  Communicate, collect, and provide information from hospitals and other organizations regarding new and existing patients with SOV home health services.
  • Monitor and follow SOV home health patients in the hospital. Relay progress and updates to SOV home health.
  • Meet with new patients and family members to collect information prior to admission and assess the living environment, family support situation.
  • Communicate with physician offices in regards to patient care and relay this information to SOV home health.
  • Visit physician offices to maintain relationships and obtain signed orders and other needed information on patients.
  • Maintain and facilitate working relationships with hospitals, physician offices, and other.
  • Answer requests for information on SOV home health services from prospective customers and others.
  • *Maintain the confidentiality of all resident, staff and organization information. 



  • Must hold a Nursing Degree from an accredited program or college/university
  • A current, unencumbered, active Ohio LPN license and current CPR
  • At least one year experience in Long Term Care
  • Experience with home care, discharge planning or insurance billing a plus.
